Submitting your first hospice medication order with Bayview Pharmacy is simple. You’ll use one of our standardized CADD order forms, fill in the required patient and prescriber information, and select the correct dosing instructions.

Once submitted, our pharmacy team prepares the medication and schedules it according to your hospice’s delivery plan.

This guide will help you understand which form to use and what steps to take so your first order is processed quickly and accurately.

Choosing the right order form

Bayview offers three medication-specific order forms for hospice CADD pump therapy. Each form collects essential dosing and titration information so our team can prepare the infusion correctly.

Here’s how to choose the form you need:

  • Hydromorphone CADD Order Form. Use this form when the patient requires hydromorphone for continuous pain control. Includes standard dosing as well as limited and full titration protocols.
  • Morphine CADD Order Form. Select this option for morphine infusions. This form mirrors the hydromorphone layout and supports both stable and titratable dosing strategies.
  • Ketamine CADD Order Form. Choose this form for ketamine infusions, typically used in complex pain or opioid-refractory cases. Features a simplified dosing structure with provider-directed adjustments.

Filling out the order form

Each form follows the same structure. Completing it accurately helps us avoid delays and ensures the patient receives medication without interruption.

  • Enter patient details. Include name, date of birth, allergies, diagnosis, and the location where the medication should be delivered (home or facility).
  • Add prescriber information. Provide the prescriber’s printed name, signature, DEA number, and office address. A signature is required before the pharmacy can dispense controlled substances.
  • Choose concentration and volume. Select the appropriate strength and bag size the prescriber is ordering. Bag size options vary by medication.
  • Specify quantity. Most hospice patients receive two bags at a time for a 96-hour supply, but you may adjust based on care needs.
  • Indicate infusion instructions.
    • Continuous rate (mg/hr)
    • PRN bolus dose and timing (if applicable)
    • Titration rules (only for hydromorphone and morphine)
    • Any special clinical considerations
  • Select supplies. Check off the infusion sets, tubing, and accessories needed for the patient. This helps ensure nurses have everything required during the visit.

Submitting the completed form

Once the form is filled out, you can send it to Bayview using the method that best fits your workflow.

  • Fax your order. Fax to 401-284-4506 for immediate processing and entry into our system.
  • Call with clinical questions. Our pharmacists at 401-284-4505 can assist if you’re unsure about dosing or titration instructions.

No matter which method you use, our team begins processing your order right away.

What happens after you submit

After your order reaches our pharmacy, several steps take place behind the scenes to ensure safe compounding and timely delivery.

  • Pharmacist review. We verify the dosing, titration limits, maximum bolus settings, bag volume compatibility, and supply selections.
  • Compounding and preparation. Your order is prepared in our sterile cleanroom according to USP <797> standards.
  • Delivery scheduling. The medication is assigned to your hospice’s delivery rotation so the patient receives their infusion bags on time.
  • Follow-up confirmations. We contact your team if clarifications are needed or once the order is officially in progress.
