Horny goat weed is a dietary supplement made from plants in the genus Epimedium. Sildenafil, sold under the brand name Viagra, is a prescription medicine the FDA approved for erectile dysfunction in 1998 after clinical trials. They are not two versions of the same thing, and the difference that matters most in practice is not the one these comparisons usually lead with.
That difference is what is actually inside the bottle. The FDA has repeatedly found undeclared prescription drug ingredients, including sildenafil, tadalafil and close chemical analogues of them, in products sold as all-natural male enhancement supplements. This page starts there, because for some readers it is the part with real consequences.
Why does the FDA keep finding sildenafil in "natural" supplements?
The FDA publishes a running list of sexual enhancement products, sold through online marketplaces and over convenience store and gas station counters, that laboratory testing shows contain undeclared prescription drug ingredients. The hidden ingredient is most often sildenafil, tadalafil, or an analogue of one of them. Products are recalled over this on an ongoing basis, and the agency describes it as public health fraud (FDA sexual enhancement product notifications).
Consider what that means for someone who picked a supplement specifically to avoid a prescription drug. They may be taking that drug anyway, in an unknown amount, without knowing it, and therefore without the warnings that would have arrived on a prescription label.
The nitrate warning is the one that matters here. PDE5 inhibitors are contraindicated with nitrate medications such as nitroglycerin and isosorbide, because the combination can cause a severe drop in blood pressure. Somebody taking a nitrate for chest pain who swallows an adulterated capsule has no way to know they have combined the two, and no label telling them not to.
The practical takeaway is narrow and worth stating plainly: "natural" on a supplement label is a marketing word, not a statement about contents, and the retail channel a product came from is not a reliable filter either. If you take nitrates, or any cardiac medication, this is worth raising with the prescriber who manages them.
If you have been taking one of these products and you also take a nitrate, a nitrate-containing chest pain medication, or an alpha blocker, that is worth mentioning to your prescriber even if nothing has gone wrong. Bring the bottle. The label will not tell them what is in it, but the brand name may appear on the FDA notification list.
What is horny goat weed, and what is icariin?
Horny goat weed is the common name for plants in the genus Epimedium, used in traditional Chinese medicine over a long period. The constituent usually named in the supplement literature is icariin, a flavonoid compound.
Most of the published interest in icariin comes from laboratory and animal work. Human evidence in erectile dysfunction is thin. There are no large randomized controlled trials establishing an effect in men with erectile dysfunction, and the human data that exists is small in scale. This page does not tell you that horny goat weed does anything, and it is not offered here as an option.
In the United States, epimedium is sold as a dietary supplement. That is a legal category and it carries a specific consequence: dietary supplements are not FDA-approved to diagnose, treat, cure, or prevent any disease, and marketing them as doing so is not lawful. Erectile dysfunction is a medical condition, so a supplement cannot properly be presented as a treatment for it.
Supplement contents also vary between manufacturers and between production runs, because there is no requirement that a bottle labelled with an icariin amount be verified to contain it. Two products with the same name on the front can differ in what they hold. That is a separate issue from deliberate adulteration, and it applies to the honest products as much as the fraudulent ones.
What is sildenafil, and why does it need a prescription?
Sildenafil is a PDE5 inhibitor. Its mechanism is described in the approved labelling, its dosing ranges are established, its adverse effects were catalogued in the trials that supported approval, and its interactions are documented, the nitrate contraindication most importantly of all. The prescription requirement is not an administrative hurdle. It is the mechanism by which somebody qualified reviews your cardiac history, your blood pressure medications and anything else you take before a PDE5 inhibitor enters the picture. That review is the thing a supplement purchase skips, and it is not a small omission.
None of this is an argument that a PDE5 inhibitor is right for you. It is contraindicated for some people, and the reasons a prescriber might rule it out are the reasons the review exists in the first place.
How do horny goat weed and sildenafil compare?

One clarification on the evidence row. "No controlled trials" describes the human erectile dysfunction literature on icariin specifically, not the whole of the laboratory record, which is larger. The distinction matters because supplement marketing frequently cites the laboratory work as though it were clinical evidence.
Is erectile dysfunction ever a sign of something else?
Often, yes. Erectile dysfunction is associated with cardiovascular disease, diabetes, hormonal factors and psychological ones, and it can be an adverse effect of medications a person is already taking. It is frequently the symptom that reaches a clinician first.

How fast does sildenafil work?
For the FDA-approved conventional tablets, labeling describes onset usually within 30 to 60 minutes with effects lasting about 4 to 6 hours, and a heavy or fatty meal can delay onset. Bayview's compounded sildenafil troches and rapid-dissolve tablets have not been evaluated by FDA for onset, duration of effect, bioavailability, or therapeutic equivalence to the approved tablets. Sexual stimulation is still needed; the medication enables the response rather than causing an erection on its own.
Are there side effects?
Most side effects are mild and short-lived: headache, facial flushing, a stuffy nose, or mild indigestion. Seek care right away for an erection lasting more than 4 hours, or sudden changes in vision or hearing. Never combine sildenafil with nitrates; the combination can drop blood pressure dangerously.

Why is tadalafil called the weekend pill?
The nickname comes from the FDA-approved conventional tadalafil tablets, whose labeling supports activity for up to 36 hours. It still requires sexual stimulation, and the duration is a window of readiness, not a continuous effect. Bayview's compounded tadalafil troches and rapid-dissolve tablets have not been evaluated by FDA for duration of effect, bioavailability, or therapeutic equivalence to the approved tablets, so no duration claim is made for them.
How is it different from sildenafil?
They work the same way, blocking the PDE5 enzyme, with similar onset and duration. The practical difference is individual response: some men get better results or fewer side effects from one than the other, so vardenafil is a common next step when sildenafil has not been the right fit.
Who should not take vardenafil?
Anyone taking nitrates or riociguat, and anyone allergic to vardenafil. Vardenafil can slightly prolong the QT interval, so tell your provider about heart rhythm conditions such as long QT syndrome or medications that affect heart rhythm. Also share any history of serious heart disease, recent stroke, or low blood pressure.
Is it approved for erectile dysfunction?
Not in the United States. The FDA-approved apomorphine products are an injection and an infusion for Parkinson's disease; a sublingual film for Parkinson's was withdrawn in 2023. The 2 mg and 3 mg sublingual tablets were approved in Europe in 2001 for erectile dysfunction, and the trial evidence for the 3 mg dose comes from that program. In the United States the use is off-label, and only your prescriber can decide whether it is appropriate for you.
What are compounded erectile dysfunction injections?
Compounded erectile dysfunction injections are sterile prescription medications injected directly into the side of the penis to help produce an erection. Depending on the prescription, the injection may contain one medication, such as alprostadil, or a combination of medications, such as papaverine, phentolamine, alprostadil, and/or atropine.
