Sildenafil works by inhibiting an enzyme called phosphodiesterase type 5 (PDE5). This enzyme is responsible for breaking down a substance called cyclic guanosine monophosphate (cGMP). cGMP is a chemical messenger that relaxes the smooth muscles of the penis, allowing for increased blood flow and an erection. By inhibiting PDE5, sildenafil helps to maintain higher levels of cGMP, thus allowing for improved erectile function.
In addition to its use in treating erectile dysfunction, sildenafil is also used to treat pulmonary hypertension. This is a condition in which the blood vessels in the lungs become narrowed, leading to increased blood pressure in the lungs. By inhibiting PDE5, sildenafil helps to relax the blood vessels in the lungs, allowing for improved blood flow and reduced blood pressure.
Sildenafil is also used to treat Raynaud's phenomenon, a condition in which the blood vessels in the fingers and toes become narrowed, leading to decreased blood flow and coldness in the extremities. Sildenafil is also used to treat benign prostatic hyperplasia (BPH), a condition in which the prostate gland becomes enlarged. By inhibiting PDE5, sildenafil helps to relax the smooth muscles of the prostate, allowing for improved urinary flow and reduced symptoms.
Finally, sildenafil is also used to treat priapism, a condition in which the penis remains erect for an extended period of time. By inhibiting PDE5, sildenafil helps to reduce the amount of cGMP in the penis, allowing for improved blood flow and reduced symptoms.

Compounded preparations have no NDC, so this formula will not appear in your EHR's drug search. Use your system's compound, non-formulary, or free-text prescription entry and name the preparation exactly as written in step 3. If your EHR forces you to pick a listed drug, pick the closest entry for the active ingredient and put the full formula in the prescription body (the Sig or directions field), not in Notes to Pharmacy.
Compounded preparations have no NDC and will not appear in your drug search. For EPCS to validate, select a drug entry your EHR recognizes as controlled, usually the bulk powder entry for the active ingredient (for example Ketamine (bulk) powder, not a branded product), then put the full formula from step 3 in the prescription body. A free-text entry that does not validate as controlled will be rejected by your EPCS module.
