Drug Interactions

A small arrow pointing down

Diclofenac Sodium 5%/Gabapentin 5%/Amitriptyline HCl 2% Topical Gel is a compounded formulation designed for the management of various pain conditions such as neuropathic pain, fibromyalgia, migraine, osteoarthritis, and rheumatoid arthritis. This semisolid formulation is dispensed through a pump mechanism, allowing for easy and precise application on the skin, targeting the treatment area directly and facilitating the absorption of active ingredients.

The formulation combines three active ingredients, each with a unique mechanism of action. Diclofenac Sodium, a non-steroidal anti-inflammatory drug (NSAID), works by inhibiting the production of certain chemicals that cause inflammation and pain. Gabapentin, an anticonvulsant, functions by decreasing abnormal excitement in the brain. Amitriptyline HCl, a tricyclic antidepressant, increases the amount of serotonin and norepinephrine in the brain, which aids in pain reduction.

However, patients using this formulation should be closely monitored for potential adverse reactions. These may include skin irritation, rash, or other allergic reactions. Additionally, due to the inclusion of an NSAID in the formulation, patients should be observed for signs of gastrointestinal bleeding. The presence of a tricyclic antidepressant also necessitates monitoring for signs of serotonin syndrome.

It is crucial to consult with a healthcare provider or pharmacist for any questions or concerns regarding this compounded formulation. They can provide comprehensive information about the medication, its use, potential side effects, and any necessary precautions.

Compounded preparations have no NDC, so this formula will not appear in your EHR's drug search. Use your system's compound, non-formulary, or free-text prescription entry and name the preparation exactly as written in step 3. If your EHR forces you to pick a listed drug, pick the closest entry for the active ingredient and put the full formula in the prescription body (the Sig or directions field), not in Notes to Pharmacy.

Compounded preparations have no NDC and will not appear in your drug search. For EPCS to validate, select a drug entry your EHR recognizes as controlled, usually the bulk powder entry for the active ingredient (for example Ketamine (bulk) powder, not a branded product), then put the full formula from step 3 in the prescription body.
