Hair loss, most often pattern hair loss (androgenetic alopecia), is the gradual thinning of hair on the scalp. It is the most common cause of hair loss in both men and women, it runs in families, and it becomes more common with age. In men it usually shows up as a receding hairline or thinning at the crown. In women it more often appears as diffuse thinning and a widening part.
It develops slowly, over years. Why does hair loss happen?
Pattern hair loss comes down to genetics and hormones. Inherited sensitivity in the hair follicles causes them to shrink and produce finer, shorter hairs until they stop growing. In men, a form of testosterone called DHT is a key driver. In women, hormonal changes, including those around menopause, can contribute alongside genetics.

Most of our formulas are built around minoxidil, which prolongs the hair's active growth phase and supports blood flow to the scalp (MedlinePlus). From there we compound in targeted ingredients chosen for the patient and the stage of thinning. Our minoxidil and finasteride formulas cover most of that range in one prescription.
Formulas for men can pair minoxidil with finasteride or dutasteride to lower the DHT behind male pattern hair loss. That ranges from a once-daily minoxidil and finasteride capsule to a high-strength minoxidil 15% foam.
Formulas for women can pair minoxidil with anti-androgen or hormonal ingredients such as spironolactone, estradiol, or progesterone for female pattern hair loss. That ranges from a gentle minoxidil and tretinoin gel to combination scalp solutions. Small amounts of tretinoin are often added to help the scalp absorb minoxidil. Where a prescriber wants an anti-inflammatory in the mix, compounded tacrolimus can be combined into a scalp preparation.

What to expect
Hair treatments work gradually. It usually takes several months of consistent daily use before the effect can be judged, and some people notice extra shedding early on as the follicles reset. Benefits hold only while treatment continues. If you stop, hair that regrew is generally lost over the following months. Results vary from person to person, and your provider may adjust your formula over time.
What to know before you start
Which of these apply depends on which formula your provider chooses. The first one matters even if the prescription is not yours.
- Finasteride and dutasteride can cause birth defects, and that risk includes handling them. Women who are pregnant or who may become pregnant should not handle these formulas at all, even to pass them to someone else. If a formula containing either is in your home, keep it somewhere that cannot happen by accident.
- Formulas containing spironolactone should not be used in pregnancy or while breastfeeding, and your provider may want to check your potassium while you are taking it.
- This takes months, and it often looks slightly worse first. Extra shedding early on is common and does not mean the treatment is failing. Judging whether it is working takes several months, not several weeks.
- Results last only as long as you keep using it. Hair regrown on treatment is generally lost over the months after stopping, so this is an ongoing commitment rather than a course of treatment.

Clinical details for prescribers
Line overview. Compounded preparations pair topical or oral minoxidil (anagen prolongation, increased follicular perfusion) with agents selected by sex and severity:
- Men's formulas. Add the 5-alpha-reductase inhibitors finasteride or dutasteride to lower scalp DHT.
- Women's formulas. Use anti-androgen or hormonal agents: spironolactone, estradiol, or progesterone.
- Penetration enhancer. Low-dose tretinoin improves minoxidil absorption across the stratum corneum.
Counseling points:
- Finasteride and dutasteride are teratogenic. They must not be handled by women who are or may become pregnant.
- With spironolactone, counsel on potassium. Avoid in pregnancy or lactation.
- Months-long time course before effect can be judged. Expect early transient shedding.
- Benefit holds only while treatment continues. Regrowth is lost on discontinuation.
