What is Tadalafil Oral Dissolve Tablets?

Tadalafil Oral Dissolve Tablet is a compounded, prescription-only medication prepared by a licensed pharmacy. It is designed to be placed under the tongue, where it dissolves and the medication is absorbed through the lining of the mouth (the oral mucous membranes) rather than being swallowed and passing through the digestive tract first. Its active ingredient is tadalafil, a medication in the phosphodiesterase type 5 (PDE5) inhibitor class.

It is one option a provider may consider as part of care for erectile dysfunction (ED). Because it is compounded, it is prepared to the strength and form your provider orders rather than mass-produced. Bayview compounds this as part of our erectile dysfunction compounding line.

How does it work?

Tadalafil is a selective PDE5 inhibitor. During sexual stimulation, it helps relax the smooth muscle and widen the blood vessels in the penis, which increases blood flow and can support an erection (MedlinePlus monograph). It works only with sexual stimulation, and it does not cure erectile dysfunction or increase sexual desire. Compared with some other PDE5 inhibitors, tadalafil has a longer duration of action.

This formulation is made as a tablet that dissolves under the tongue so the medication is taken up through the oral mucous membranes.

What is it used for?

Providers may prescribe tadalafil as part of care for erectile dysfunction when they determine it is appropriate for a patient. PDE5 inhibitors are among the oral medications described for ED in the American Urological Association ED guideline. Only a licensed provider can decide whether this formulation is right for an individual; Bayview Pharmacy does not provide consultations or determine appropriateness, and a valid prescription is required.

How does it compare to other ED options?

ED treatments differ mainly in how they are taken and how long they last. The table below describes those differences for education only.

How is it used?

Place the tablet under your tongue and allow it to dissolve fully, do not chew it or swallow it whole, and follow the exact timing and dose set by your prescriber. Tadalafil may be prescribed either as needed before sexual activity or as a regular low daily dose, depending on what your provider decides. Your prescriber sets your dose and schedule, so never change your dose on your own. A Bayview pharmacist is available if you have questions about your prescription.

Side effects & safety

As with any medication, side effects are possible. Reported effects of PDE5 inhibitors can include headache, flushing, stuffy or runny nose, indigestion, and dizziness. Back pain and muscle aches are also reported with tadalafil and usually ease on their own. Some people notice temporary vision changes such as blurred vision or increased light sensitivity. Report any of these to your provider.

Do not use with nitrates or riociguat. Taking tadalafil with nitrate medications (such as nitroglycerin or isosorbide, often used for chest pain/heart conditions) or with riociguat (Adempas) can cause a sudden, dangerous drop in blood pressure. Tell your provider about every medication you take before using this product.
Seek emergency care for a prolonged erection. An erection lasting more than 4 hours (priapism) is a medical emergency and can cause permanent damage if it is not treated promptly. Go to the emergency room or seek immediate medical care. Learn more from the Urology Care Foundation.

Stop and seek medical care right away if you have a sudden loss of vision or hearing, chest pain, or a reaction that concerns you.

Who should not use it?

Tadalafil is not appropriate for everyone. It should not be used by people taking nitrate medications or riociguat, and extra caution applies to those taking alpha-blockers or other blood-pressure medications (the combination can lower blood pressure too much), and to those with certain heart conditions or for whom sexual activity is inadvisable. Tell your provider about your health conditions and all medications and supplements you take so they can determine whether this formulation is appropriate for you.

How should it be stored?

Orally dissolving tablets are sensitive to moisture. Store exactly as directed on your label, keep the tablets dry, in their original packaging until use, away from heat, light, and humidity, and out of the reach of children. Do not use a tablet that has become damaged or damp.

Clinical details for prescribers

Mechanism & form. Tadalafil is a selective PDE5 inhibitor that potentiates cGMP-mediated cavernosal smooth-muscle relaxation in response to nitric-oxide release during sexual stimulation. Its notably longer half-life than other PDE5 inhibitors supports both as-needed and low-dose daily regimens. This compounded formulation is a transmucosal (sublingual) orally dissolving tablet. Contraindications & interactions:

  • Nitrates, nitric-oxide donors, or riociguat, contraindicated (risk of profound hypotension)
  • Additive hypotension with alpha-blockers and antihypertensives
  • Consider hepatic/renal function and concomitant CYP3A4 inhibitors

  • Dose timing (as-needed vs. daily)
  • Characteristic back/muscle pain
  • Priapism precaution: erection >4 hours is an emergency

Compounded medications are not FDA-approved. They're prepared by a licensed pharmacy pursuant to a valid prescription for an individual patient, under USP standards. The FDA does not review compounded medications for safety or effectiveness.

Can you fill a prescription from my own provider?

Yes. Your provider can send your prescription to Bayview Pharmacy by e-prescribe, fax, or phone, or you can transfer an existing prescription from another pharmacy.

Do I need a prescription?

Yes. All of our medications require a valid prescription from a licensed provider. We're a dispensing and compounding pharmacy, so we don't provide consultations or write prescriptions. If you don't have one yet, you'll need to find and see a provider first.

How do providers send a prescription to Bayview?

Providers can submit a prescription three ways:

  • E-prescribe: Search Bayview Pharmacy, Warwick, RI (NCPDP 4106882) in your EHR. For compounds, use the compound, custom, or free-text option and include the full formulation, strength, quantity, refills, and directions.
  • Fax: Send a signed order to 401-284-4506 (alt. 401-210-2757) with patient and prescriber details, medication, quantity, refills, directions, signature, and date.
  • Phone: Call 401-284-4505 to submit, clarify, or coordinate refills.

A valid prescription is required, and compounded medications are not FDA-approved.

How do refills and automatic refills work?

Refills are coordinated with your prescriber and your prescription. You can request a refill through our website or by calling us, and we'll handle the rest as long as you have refills remaining on a valid prescription. You can also opt into automatic refills on non-controlled substances, with no need to worry about running out of your medication. We can either ship it to you automatically, or ready for pickup.

How does shipping work?

Once your prescription is verified, we prepare your medication and ship it discreetly in plain packaging either via UPS or USPS. A pharmacy technician confirms the details with you before it goes out, and we're available if you have any questions.

What is compounding?

Compounding is the practice of preparing a medication tailored to an individual patient's prescription. Instead of a mass-produced product, a compounding pharmacy makes the medication to the specific strength, form, or combination your provider orders.

Examples on this site include medications compounded as liquids for people who cannot swallow pills, thyroid capsules made to a T3/T4 ratio no manufactured product offers, and all-purpose nipple ointment, which combines three actives that are not sold together commercially.

That long window is tadalafil's signature difference from sildenafil and vardenafil. It still requires sexual stimulation, and the duration is a window of readiness, not a continuous effect.

Can I take it every day?

Yes, if your provider prescribes it that way. A low daily dose keeps a steady level in your system so spontaneity never depends on timing a pill, while on-demand dosing before activity suits men who use it less often. Both approaches use the same medication at different strengths; your provider chooses based on your pattern and health.

How does compounded tadalafil compare to the FDA-approved tablet?

Cialis® and its generics are FDA-approved conventional tablets. Bayview dispenses those manufactured tablets, and separately compounds tadalafil as cheek-and-gum troches and under-the-tongue rapid dissolve tablets in the strength your provider specifies. Compounded preparations are not FDA-approved and have not been evaluated by FDA for safety or effectiveness. Your prescriber decides which form is appropriate for you.

Who should not take tadalafil?

Anyone taking nitrates or riociguat, or allergic to tadalafil. Because it lasts up to 36 hours, the nitrate interaction lasts longer. Tell your provider about heart conditions.

What forms can I get?

Swallowed tablets, cheek-and-gum troches, and under-the-tongue rapid dissolve tablets, each in the strength your provider chooses. The troche and rapid dissolve forms are taken without water, which many men prefer for discretion and convenience.
