Budesonide is a potent corticosteroid with anti-inflammatory properties, commonly utilized in compounded formulations to manage various inflammatory conditions.
Budesonide is a glucocorticoid steroid with anti-inflammatory properties, used in the management of a range of inflammatory conditions. FDA-approved uses include respiratory disorders such as asthma and chronic obstructive pulmonary disease (COPD), allergic rhinitis, and inflammatory bowel diseases such as Crohn's disease and ulcerative colitis.[1] Budesonide works by binding and activating glucocorticoid receptors, which prevents the production of inflammatory genes.[1]
In compounded preparations, Budesonide can be prepared on a prescriber's order in dosage forms or concentrations that are not available in commercially manufactured products. High-volume sinonasal budesonide irrigation is an off-label use that is commonly used to manage chronic rhinosinusitis.[1]
When budesonide is swallowed, it undergoes high first-pass metabolism (80 to 90%), mainly by CYP3A4, to metabolites with negligible glucocorticoid activity.[2] Adverse effects can still occur, including oral or throat candida infections with inhaled use, reduced growth velocity in children, and hypothalamic-pituitary-adrenal (HPA) axis suppression.[1]

They are compounded, prescription capsules that hold a measured dose of budesonide, a corticosteroid, in a dissolvable powder. You open a capsule and mix the powder into a saline sinus rinse, such as a NeilMed bottle, or a nebulizer, so the anti-inflammatory medication reaches the lining of your nose and sinuses. Bayview most often makes these at 0.6 mg.
