Levothyroxine Sodium is a synthetic form of the thyroid hormone thyroxine, primarily used to treat hypothyroidism by supplementing low levels of thyroid hormone in the body.
Levothyroxine sodium is a synthetic version of the body's natural thyroid hormone thyroxine (T4).[1] Thyroid hormones control the way the body uses energy.[2] Levothyroxine is FDA approved to treat hypothyroidism,[1] a condition in which the thyroid gland does not make enough thyroid hormones to meet the body's needs, with symptoms that can include fatigue, weight gain, and depression.[2]
Levothyroxine sodium is typically administered orally, and its absorption can be influenced by food and by other medicines. It is given on an empty stomach, 30 to 60 minutes before breakfast or 3 to 4 hours after dinner, and it should be separated from iron, calcium, antacids, and proton pump inhibitors; dietary fiber reduces its bioavailability.[1] Prescribers should consider these factors when determining the dosage and administration schedule.
In addition to hypothyroidism, levothyroxine is FDA approved for thyrotropin (TSH) suppression in well-differentiated thyroid cancer, as an adjunct to surgery and radioiodine therapy.[1]

Standard first-line therapy for hypothyroidism is a commercial levothyroxine product. Compounding is used when a provider wants a custom dose or T3:T4 ratio, or a capsule without certain dyes, fillers, or allergens. Compounded thyroid is not FDA-approved.
