Phentolamine Mesylate is a potent non-selective alpha-adrenergic antagonist widely utilized in various compounded formulations.
Phentolamine mesylate is a reversible, nonselective alpha-adrenergic blocker.[2] It produces an alpha-adrenergic block of relatively short duration and also has direct vasodilator properties; its half-life in the blood is about 19 minutes after intravenous administration.[1]
FDA-approved phentolamine injection is indicated for the prevention or control of hypertensive episodes that may occur in a patient with pheochromocytoma, and for the prevention or treatment of dermal necrosis following extravasation of norepinephrine.[1]
It is also labeled for the diagnosis of pheochromocytoma by the phentolamine blocking test, although biochemical assays have largely replaced the phentolamine test and other pharmacological tests for reasons of accuracy and safety.[1] Phentolamine is a component of Trimix, a compounded combination of alprostadil, papaverine, and phentolamine used off-label for intracavernosal injection therapy in erectile dysfunction.[2][3]
Prescribers should be aware of potential side effects, including acute and prolonged hypotensive episodes, tachycardia, cardiac arrhythmias, weakness, dizziness, flushing, nausea, vomiting, and diarrhea.[1] Myocardial infarction, cerebrovascular spasm, and cerebrovascular occlusion have been reported after phentolamine administration, usually in association with marked hypotensive episodes.[1]
