Alprostadil is a form of prostaglandin E1 that Bayview compounds to a prescriber's order.
Alprostadil is a synthetic prostaglandin E1 (PGE1) analog and a vasodilator.[1] It is FDA-approved as a second-line treatment for erectile dysfunction, given by intracavernosal injection or intraurethral suppository, and in its intravenous form it is FDA-approved to temporarily maintain patency of the ductus arteriosus in neonates.[1]
Alprostadil binds as an agonist to prostaglandin receptors (for example, EP2), activating adenylate cyclase and increasing cyclic adenosine monophosphate (cAMP). This leads to smooth muscle relaxation and vasodilation, increasing peripheral and intracavernosal blood flow.[1] Alprostadil can also inhibit platelet aggregation and has antioxidant properties.[1]
Common adverse effects include penile pain, hypotension, flushing, and priapism.[1] Bayview compounds alprostadil alone and in combination injections and a urethral gel on a prescriber's order.

The main difference is the combination of active ingredients. Alprostadil contains a single active medication. BiMix usually contains two medications. TriMix usually contains three. Atropine TriMix includes atropine in addition to a TriMix-style formula, and QuadMix generally contains four medications. Your prescriber chooses the formulation based on your medical history, response to treatment, and tolerance.
