Tobramycin Sulfate is a potent aminoglycoside antibiotic widely utilized in compounded formulations for its efficacy against a broad spectrum of gram-negative bacterial infections.
Tobramycin sulfate is an aminoglycoside antibiotic active against aerobic gram-negative bacteria, including Pseudomonas aeruginosa, Escherichia coli, and Klebsiella species, as well as Staphylococcus aureus.[1] It is bactericidal and works by binding to the 16S ribosomal RNA of the bacterial 30S ribosomal subunit, inhibiting the initiation of protein synthesis.[1]
FDA-approved systemic indications include respiratory tract, urinary tract, and skin and subcutaneous tissue infections, among others.[1] Tobramycin 0.3% ophthalmic solution is indicated for external infections of the eye and its adnexa caused by susceptible bacteria.[2] Tobramycin has poor oral bioavailability and is given by intravenous, intramuscular, topical, or inhaled routes.[1]
Prescribers should be aware of the potential nephrotoxic and ototoxic effects associated with systemic tobramycin, which carry boxed warnings.[1] Monitoring of serum drug levels and renal function is recommended.[1] When topical ocular tobramycin is used together with systemic aminoglycosides, total serum concentration should be monitored.[2] Tobramycin can be combined with beta-lactams, such as penicillins or cephalosporins, with which it works synergistically against gram-negative bacteria.[1]

"Fortified" means the eye drop is compounded at a much higher concentration than the standard commercial tobramycin drop, which is 0.3% (3 mg/mL). Bayview compounds it at 15 mg/mL and 13.6 mg/mL, roughly five times the commercial strength, because a serious eye infection can need more antibiotic delivered to the surface of the eye.
It is used to treat serious bacterial eye infections, most often bacterial keratitis (a corneal infection or ulcer), especially when gram-negative bacteria such as Pseudomonas are involved or suspected. An eye specialist prescribes and monitors it, often based on cultures of the infection.
