Vancomycin Hydrochloride is a potent antibiotic primarily used to treat severe bacterial infections caused by Gram-positive bacteria, including methicillin-resistant Staphylococcus aureus (MRSA).
Vancomycin hydrochloride is a glycopeptide antibiotic used to treat infections caused by gram-positive bacteria, including methicillin-resistant Staphylococcus aureus (MRSA).[1] It works by binding to D-alanyl-D-alanine and inhibiting the polymerization of peptidoglycans in the bacterial cell wall.[1]
In compounding, vancomycin is prepared on a prescriber's order for routes and strengths that are not commercially available. For example, fortified vancomycin 5% eye drops, which are very active against MRSA, are used off-label as a topical option in the treatment of bacterial keratitis.[2]
Systemic vancomycin is used for serious gram-positive infections, including endocarditis, osteomyelitis, and skin and soft tissue infections, and oral vancomycin is used for Clostridioides difficile-associated diarrhea.[1]
Prescribers should be aware of the potential for nephrotoxicity, including acute kidney injury, and for transient or permanent ototoxicity, which can present as hearing loss, tinnitus, or vertigo.[1] Therapeutic drug monitoring with serum trough concentrations, or AUC monitoring for serious infections, is recommended.[1] Intravitreal vancomycin has been associated with hemorrhagic occlusive retinal vasculitis causing permanent vision loss.[1]
