4-Aminopyridine, also called fampridine, is a potassium channel blocker that Bayview compounds into sustained-release capsules and an oral suspension on a prescriber's order.
4-Aminopyridine (4-AP), also called fampridine, is a potassium channel blocker. Its FDA-approved form is dalfampridine extended-release 10 mg tablets, approved in 2010 to improve walking in adults with multiple sclerosis, as shown by an increase in walking speed.[1][2] How it works is not fully understood; in animal studies, blocking potassium channels helped damaged (demyelinated) nerve fibers carry signals.[1]
Neurologists also prescribe 4-aminopyridine off-label, based on small studies, for conditions such as downbeat nystagmus and episodic ataxia type 2.[3][4] The FDA-approved medicine for Lambert-Eaton myasthenic syndrome is amifampridine (3,4-diaminopyridine), a different drug.[5] Only a licensed prescriber can decide whether 4-aminopyridine is appropriate.
Seizures are the main risk, and higher doses and reduced kidney function raise it. People who have had a seizure or who have moderate or severe kidney disease should not take it, and the dalfampridine tablet's label says not to take it with any other form of 4-aminopyridine, including compounded 4-AP.[1] Too much 4-aminopyridine can cause seizures, confusion, and shaking.[1][6]
