Betahistine Dihydrochloride is a histamine analog used to relieve vertigo associated with Ménière's disease.[1] It is thought to work by increasing circulation in the inner ear and reducing endolymphatic pressure.[1][2] Evidence for benefit is limited: a Cochrane review found low-quality evidence that betahistine may reduce vertigo symptoms,[3] and a NICE evidence review found no clinically important difference between betahistine and placebo in quality of life for people with Ménière's disease.[2]
Betahistine acts as a weak histamine H1 receptor agonist and a strong H3 receptor antagonist.[1][2] Stimulation of H1 receptors in the inner ear causes vasodilation, and H3 antagonism increases histamine turnover.[1] Betahistine is generally well tolerated with a low risk of adverse events; the most common side effects are gastrointestinal symptoms and headache.[3]
