Hydroxocobalamin is a highly bioavailable form of vitamin B12, essential for numerous physiological functions, including DNA synthesis, red blood cell formation, and neurological health.
Hydroxocobalamin, a form of vitamin B12, is a crucial active pharmaceutical ingredient used in various compounded formulations. It plays a vital role in the body's metabolic processes, particularly in DNA synthesis and red blood cell formation. As a highly bioavailable form of vitamin B12, hydroxocobalamin is often preferred for its longer retention in the body compared to other forms like cyanocobalamin.
Clinically, hydroxocobalamin is utilized to treat vitamin B12 deficiency, which can lead to conditions such as pernicious anemia and neurological disorders. Its efficacy in replenishing B12 levels makes it a valuable option for patients with absorption issues or dietary restrictions. Additionally, hydroxocobalamin is used as an antidote for cyanide poisoning due to its ability to bind cyanide ions, forming a non-toxic compound that is excreted from the body.
In the context of compounding, hydroxocobalamin's stability and effectiveness make it a versatile ingredient for personalized medicine. It can be tailored to meet specific patient needs, ensuring optimal therapeutic outcomes. Prescribers often choose hydroxocobalamin for its superior pharmacokinetic profile, which supports sustained release and prolonged action in the body.
Furthermore, hydroxocobalamin is generally well-tolerated, with a low incidence of adverse effects. This makes it suitable for a wide range of patients, including those with sensitivities to other forms of vitamin B12. Its use in compounded formulations allows for customization in dosage and delivery method, enhancing patient compliance and satisfaction.

They are three forms of the same vitamin. Cyanocobalamin is the FDA-approved one in the United States, both as an injection and as the Nascobal nasal spray. Methylcobalamin and hydroxocobalamin have no FDA-approved vitamin B12 product here, so they are compounded when a prescriber orders them.
High-quality evidence does not show the methyl or hydroxo forms work better. Claims of superiority for those forms are not well supported.

Compounded preparations are not FDA-approved, and the FDA has not evaluated them for safety, effectiveness, or manufacturing quality. Nothing on this page is a claim that any preparation is effective for any condition.
