Hydroxocobalamin is a form of vitamin B12 that Bayview compounds to a prescriber's order.
Hydroxocobalamin is a manufactured, injectable form of vitamin B12.[1] Vitamin B12 cofactors are involved in DNA and amino acid synthesis and support red blood cell formation.[1] Hydroxocobalamin is given by intramuscular or intravenous injection, and it requires less frequent administration (every three months) than oral cyanocobalamin supplementation.[1]
Clinically, hydroxocobalamin is used to treat vitamin B12 deficiency and the macrocytic anemia it causes.[1] It is particularly useful when B12 deficiency results from conditions that impair absorption from the gastrointestinal tract, including malabsorption and pernicious anemia.[1] Hydroxocobalamin is also used as an antidote for cyanide poisoning, because its cobalt binds cyanide to form cyanocobalamin, which is excreted by the kidneys in urine.[1]

Reported adverse effects include rash, itching, fever, nausea, dizziness, and hot flushes, and anaphylaxis is rare.[1]

They are three forms of the same vitamin. Cyanocobalamin is the FDA-approved one in the United States, both as an injection and as the Nascobal nasal spray. Methylcobalamin and hydroxocobalamin have no FDA-approved vitamin B12 product here, so they are compounded when a prescriber orders them.
High-quality evidence does not show the methyl or hydroxo forms work better. Claims of superiority for those forms are not well supported.
