Naltrexone hydrochloride is an opioid antagonist that is FDA-approved for the treatment of alcohol use disorder and opioid use disorder.[1] It acts mainly at mu-opioid receptors, with weaker activity at kappa and delta receptors, and it blocks the effects of opioids, preventing opioid intoxication and the euphoric reinforcement that drives substance use; in alcohol use disorder it reduces heavy drinking and cravings.[1]
A fixed-dose combination of naltrexone with bupropion is also FDA-approved for obesity management.[1] Low-dose naltrexone (LDN), generally 1 to 5 mg daily, remains an off-label option.[2] It has been studied in conditions such as fibromyalgia, multiple sclerosis, and Crohn's disease, with proposed mechanisms that include antagonism of Toll-like receptor 4 on microglia and transient opioid receptor blockade, but current evidence is limited and consists mostly of small studies.[2]
When considering naltrexone, it is important to review the patient's medical history and current medications. Naltrexone is contraindicated in patients currently using or dependent on opioids, in acute opioid withdrawal, and in those who fail a naloxone challenge test, and baseline and periodic liver function tests are recommended.[1] Dosing varies with the condition being treated.

Low Dose Naltrexone, often called LDN, is naltrexone prepared in much smaller doses than the commercially available 50 mg tablet. Because these low strengths are not sold as a standard manufactured product, they are commonly prepared by a compounding pharmacy with a prescription.

Compounded preparations are cash-priced and are generally eligible for HSA and FSA cards. From a licensed pharmacy, with a prescription. LDN is not sold over the counter and it is not a manufactured product at these strengths, so it has to be prepared by a compounding pharmacy. Bayview is licensed in Rhode Island, Massachusetts, Connecticut, New Jersey, New Hampshire, Florida, Maine and Pennsylvania, and currently serves eligible New York patients on a limited basis under New York's isolated-transaction criteria while full nonresident pharmacy registration is pending. Naltrexone is commercially manufactured only as a 50 mg tablet. There is no mass-produced 1.5 mg or 4.5 mg product, so low strengths have to be prepared by a compounding pharmacy. That is the only reason it is compounded, not because a compounded version is different or better. Do not try to split or dissolve a 50 mg tablet at home; the accuracy is not there and no stability data supports it.
