Sildenafil (as sildenafil citrate) is the active ingredient in FDA-approved conventional oral tablets.[1] Bayview also uses sildenafil in patient-specific compounded troches and rapid-dissolve tablets when prescribed for an individual patient. Bayview's compounded sildenafil preparations are not FDA-approved and have not been reviewed by FDA for safety, effectiveness, quality, bioavailability, or therapeutic equivalence to approved sildenafil tablets.[3]
Sildenafil is a phosphodiesterase type 5 (PDE5) inhibitor. It enhances the effect of nitric oxide, relaxing smooth muscle and supporting blood flow to the penis during sexual stimulation; it does not cause an erection on its own.[1] FDA-approved sildenafil products are also labeled, at different strengths, for pulmonary arterial hypertension.[2] Only a licensed prescriber decides whether sildenafil, and which dosage form, is appropriate for a given patient.
Prescribers should screen for nitrates and riociguat (contraindicated), additive hypotension with alpha-blockers and antihypertensives, hepatic or renal impairment, CYP3A4 inhibitors, and cardiovascular status before prescribing.[1] Published pharmacokinetic data, including onset and duration of effect, derive from the FDA-approved conventional tablets and do not automatically apply to compounded dosage forms.
