Trilostane is an adrenal steroid synthesis inhibitor used in veterinary medicine that Bayview compounds to a prescriber's order.
Trilostane is an inhibitor of the enzyme 3β-hydroxysteroid dehydrogenase in the adrenal cortex. By blocking the conversion of pregnenolone to progesterone, it suppresses the production of glucocorticoids such as cortisol, mineralocorticoids such as aldosterone, and sex hormones.[1]
In veterinary medicine, trilostane is FDA-approved (as VETORYL Capsules) for the treatment of pituitary-dependent and adrenal-dependent hyperadrenocorticism (Cushing's disease) in dogs.[1] Clinical signs of hyperadrenocorticism in dogs include increased thirst, increased urination, increased appetite, lethargy, panting, a pot-bellied appearance, thin skin, hair loss, and muscle wasting.[1]
In dogs, absorption of trilostane is enhanced when it is given with food, peak plasma levels occur within about 1.5 hours, and levels return to baseline within about 12 hours, with significant variation between dogs.[1] The approved product is contraindicated in dogs with primary hepatic or renal disease.[1]
Reported adverse reactions include poor appetite, vomiting, lethargy, diarrhea, and weakness. Hypoadrenocortical crisis or adrenal necrosis or rupture may occur and may result in death, and electrolyte changes such as elevated potassium and decreased sodium can signal a problem.[1] Monitoring with ACTH stimulation tests and serum biochemistry, including electrolytes and renal and hepatic function, is recommended during treatment.[1]
