Ursodiol is a bile acid (ursodeoxycholic acid) that Bayview compounds to a prescriber's order.
Ursodiol, also known as ursodeoxycholic acid, is a bile acid used in the management of certain liver and gallbladder conditions.[2] Ursodiol capsules are FDA-approved for dissolving radiolucent, noncalcified gallbladder stones smaller than 20 mm in patients who would otherwise have gallbladder surgery but are at increased surgical risk, and for preventing gallstone formation in obese patients undergoing rapid weight loss.[1] Ursodiol is also the only drug approved by the FDA to treat primary biliary cholangitis, a chronic cholestatic liver disease.[2]
Ursodiol suppresses hepatic synthesis and secretion of cholesterol and inhibits intestinal absorption of cholesterol, which helps solubilize cholesterol in bile and gradually dissolve cholesterol gallstones.[1] In cholestatic liver disease, it is thought to act by replacing toxic hydrophobic bile acids, protecting liver cells, and increasing bile acid secretion.[2]

Ursodiol is mostly well tolerated. Diarrhea has been reported in gallstone patients, and abdominal pain and worsening itching have been reported in patients with primary biliary cholangitis.[2] Bile acid sequestrants such as cholestyramine and colestipol, and aluminum-based antacids, can reduce ursodiol absorption, while estrogens, oral contraceptives, and clofibrate may counteract its effect on gallstones.[1] Liver enzymes (AST and ALT) should be measured at the start of therapy and thereafter as clinically indicated.[1]
