Enclomiphene citrate is a selective estrogen receptor modulator (SERM) and one of the two geometric isomers, with zuclomiphene, that make up clomiphene.[1] It acts by blocking the estrogenic suppression of the hypothalamic-pituitary-gonadal axis, and in clinical trials in men with secondary hypogonadism it raised testosterone while increasing luteinizing hormone (LH) and follicle-stimulating hormone (FSH).[1] No FDA-approved drug contains enclomiphene as its active ingredient.[1]
In clinical trials, enclomiphene achieved testosterone levels comparable to testosterone replacement while sperm production was preserved.[1] FDA reviewers noted safety concerns including cardiac and thromboembolic events, elevated estradiol, increased PSA, and increased hematocrit, and that long-term safety data are lacking.[1]
The clinical trials did not evaluate improvement in hypogonadal symptoms or quality of life, and FDA stated that it is unclear whether increasing testosterone in secondary hypogonadism confers clinical benefit.[1] In 2022, FDA's Pharmacy Compounding Advisory Committee voted 8 to 4 against placing enclomiphene citrate on the 503A bulks list, with several members citing a lack of clinical efficacy evidence.[2]
Enclomiphene has also been studied in follicular development and ovulation induction.[1]

It blocks estrogen's signal at the pituitary and hypothalamus, which is understood to increase the hormones (LH and FSH) that prompt the testes to make more of the body's own testosterone and support sperm production. It works through the body's own hormone axis rather than adding testosterone from outside.

Enclomiphene is not FDA-approved, so it has been studied less than approved medicines. Side effects reported in clinical trials include headache, abdominal discomfort, and higher estradiol (estrogen) levels. Medicines in this class can also cause hot flashes or flushing, nausea, mood changes, and breast tenderness. Vision changes such as blurring, spots, or flashes can occur with medications in this class; if you notice any change in your vision, stop taking it and contact your provider. Your provider checks your labs and symptoms at follow-up. Report any effect that concerns you.
