Phenylephrine Hydrochloride is an alpha-1 adrenergic agonist that Bayview compounds to a prescriber's order.
Phenylephrine hydrochloride is a direct-acting sympathomimetic amine and alpha-1 adrenergic agonist that causes vasoconstriction without significant beta activity, which increases systemic vascular resistance.[1] FDA-approved uses include intravenous treatment of clinically significant hypotension in adults, such as during anesthesia or in septic shock, ophthalmic use for pupil dilation, intranasal use for nasal congestion, and as an over-the-counter ingredient in hemorrhoid products.[1]
Phenylephrine applied to the nasal mucosa is used to relieve nasal congestion.[1] In 2023, FDA concluded that oral phenylephrine is not effective as a decongestant.[1]
Phenylephrine is also the preferred first-line vasoactive agent for reversal of ischemic priapism by intracorporal injection, and the dilute solution for this use can be prepared by a pharmacy.[2] Patients receiving it should have vital signs and blood pressure checked and be monitored for symptoms of hypertension.[2]
When given intravenously, phenylephrine has an onset of 1 to 3 minutes and a duration of about 5 to 20 minutes.[1] Side effects may include nausea, vomiting, headache, nervousness, and reflex bradycardia, and monoamine oxidase inhibitors, tricyclic antidepressants, and other sympathomimetics can increase its pressor effect.[1] Prescribers should also consider existing cardiovascular conditions, as increased afterload may worsen angina, heart failure, or pulmonary hypertension, and phenylephrine should be avoided in severe bradycardia.[1]
