Formula Information

A small arrow pointing down

This compounded topical spray combines Diphenhydramine HCl 10 mg/mL, Hydrocortisone Acetate 10 mg/mL, and Pramoxine HCl 10 mg/mL in a convenient liquid formulation designed for direct application to the skin. This unique combination offers synergistic effects to provide relief from various skin discomforts by addressing multiple symptoms simultaneously.

Diphenhydramine HCl is an antihistamine that works by blocking the action of histamine, a substance in the body that causes allergic symptoms. By inhibiting histamine, diphenhydramine helps to reduce itching and swelling associated with allergic reactions and irritations.

Hydrocortisone Acetate is a topical corticosteroid that reduces inflammation by suppressing the immune response in the affected area. It helps alleviate redness, swelling, and discomfort caused by inflammatory skin conditions by inhibiting the release of substances in the body that cause inflammation.

Pramoxine HCl is a local anesthetic that provides temporary relief from pain and itching by blocking nerve signals in the skin. It helps to numb the affected area, offering quick relief from discomfort associated with minor skin irritations.

The combination of these three active ingredients in a topical spray allows for easy and targeted application, ensuring that the medication is delivered precisely where it is needed. The spray form minimizes contact with the affected area, which can help prevent further irritation and promote healing.

This compounded medication is beneficial for patients who require multi-symptom relief from skin discomforts, providing antihistaminic, anti-inflammatory, and anesthetic effects in one formulation. It can be particularly useful when over-the-counter options are insufficient or when a tailored approach is needed.

For prescribers, this formulation offers a customizable option to address specific patient needs, especially in cases where patients may be sensitive to certain excipients found in commercial products or require alternative concentrations of active ingredients.

Patients should apply the spray as directed by their healthcare provider, usually to the affected area up to the prescribed number of times per day. It is important to avoid contact with the eyes and mucous membranes and to use the medication only on intact skin unless otherwise directed.

As with any medication, patients should be aware of potential side effects, which may include local skin reactions such as burning, stinging, or irritation at the application site. If any severe reactions occur, patients should discontinue use and consult their healthcare provider.

If you have any questions about this compounded topical spray or how it may benefit you or your patients, please reach out to us for more information. Our team at Bayview Pharmacy is here to assist with any inquiries regarding this formulation.

Drug Interactions

A small arrow pointing down

When using the Diphenhydramine HCl 10 mg/mL/Hydrocortisone Acetate 10 mg/mL/Pramoxine HCl 10 mg/mL Topical Spray, it is important to consider potential drug interactions, even though the formulation is applied topically. This combination spray is designed to provide relief from itching, inflammation, and pain associated with various dermatological conditions. Diphenhydramine is an antihistamine that helps reduce itching and allergic reactions, hydrocortisone is a corticosteroid that reduces inflammation, and pramoxine is a local anesthetic that helps relieve pain and itching.

While the systemic absorption of these medications is generally low when applied topically, there is still a potential for interactions, especially if the spray is used over large areas of the body or for prolonged periods. Patients should be cautious if they are using other medications that contain similar active ingredients, such as oral antihistamines, systemic corticosteroids, or other topical anesthetics, as this may increase the risk of additive side effects or toxicity.

For patients taking oral diphenhydramine or other sedating antihistamines, there is a potential for increased sedation or anticholinergic effects, such as dry mouth, urinary retention, or constipation. Patients should be advised to monitor for these symptoms and consult their healthcare provider if they experience any adverse effects. Additionally, combining multiple sources of corticosteroids, whether topical or systemic, can increase the risk of systemic corticosteroid effects, such as adrenal suppression, especially in pediatric patients or when used over large surface areas.

Pramoxine, as a local anesthetic, generally has a low risk of systemic interactions. However, patients should avoid using other local anesthetics concurrently on the same area to prevent potential additive effects, which could lead to increased local irritation or systemic absorption. Patients with a history of sensitivity to local anesthetics should use this product with caution and under the guidance of a healthcare provider.

Prescribers should evaluate the patient's current medication regimen, including over-the-counter products, to identify any potential interactions. It is also important to consider the patient's overall health status, including any pre-existing conditions that may be affected by the use of topical corticosteroids or antihistamines. Patients with conditions such as glaucoma, hypertension, or diabetes should use this formulation with caution and under medical supervision.

If you have any questions or concerns about using this formulation, or if you need further information on potential drug interactions, please reach out to us at Bayview Pharmacy. Our team of knowledgeable compounding pharmacists is here to assist you and ensure the safe and appropriate use of this medication.

How Does it Work

A small arrow pointing down

This topical spray formulation combines three active ingredients: Diphenhydramine HCl, Hydrocortisone Acetate, and Pramoxine HCl, each contributing to the relief of various skin conditions through distinct mechanisms of action. The liquid formulation is contained in a spray bottle, allowing for precise and convenient application directly to the affected area, ensuring that the medication is delivered efficiently and effectively.

Diphenhydramine HCl is an antihistamine that works by blocking the action of histamine, a substance in the body that causes allergic symptoms. When applied topically, Diphenhydramine HCl helps to alleviate itching and discomfort associated with allergic reactions, insect bites, and other skin irritations. By inhibiting the histamine response, it reduces the sensation of itchiness and provides relief from the urge to scratch, which can further irritate the skin.

Hydrocortisone Acetate is a corticosteroid that reduces inflammation by suppressing the immune response and decreasing the production of inflammatory chemicals in the body. When applied to the skin, it helps to reduce redness, swelling, and irritation associated with various dermatological conditions such as eczema, psoriasis, and dermatitis.

Pramoxine HCl is a local anesthetic that works by numbing the skin and blocking the transmission of nerve signals that cause pain and itching. This provides immediate relief from discomfort and itching, making it particularly useful for conditions that cause significant irritation and pain. By numbing the affected area, Pramoxine HCl helps to break the itch-scratch cycle, allowing the skin to heal more effectively.

The combination of these three active ingredients in a topical spray formulation offers a comprehensive approach to managing skin conditions characterized by itching, inflammation, and pain. The spray delivery system ensures that the medication is evenly distributed over the affected area, providing targeted relief without the need for direct contact with the skin, which can be beneficial for sensitive or painful areas.

It is important to follow the prescribed dosage and application instructions to achieve optimal results. If you have any questions or need further information about this formulation, please reach out to us at Bayview Pharmacy.

Compounded preparations have no NDC, so this formula will not appear in your EHR's drug search. Use your system's compound, non-formulary, or free-text prescription entry and name the preparation exactly as written in step 3. If your EHR forces you to pick a listed drug, pick the closest entry for the active ingredient and put the full formula in the prescription body (the Sig or directions field), not in Notes to Pharmacy.

Compounded preparations have no NDC and will not appear in your drug search. For EPCS to validate, select a drug entry your EHR recognizes as controlled, usually the bulk powder entry for the active ingredient (for example Ketamine (bulk) powder, not a branded product), then put the full formula from step 3 in the prescription body. A free-text entry that does not validate as controlled will be rejected by your EPCS module.
