Gentamicin Sulfate is an aminoglycoside antibiotic that Bayview compounds to a prescriber's order.
Gentamicin Sulfate is an aminoglycoside antibiotic used in the treatment of several Gram-negative infections.[1] It is bactericidal against aerobic Gram-negative organisms, including Enterobacteriaceae and Pseudomonas aeruginosa, and against certain Gram-positive bacteria when combined with a beta-lactam.[1] Gentamicin works by binding to the 30S subunit of the bacterial ribosome, disrupting protein synthesis and leading to bacterial cell death.[1]
In addition to intravenous and intramuscular use, gentamicin is available in ophthalmic preparations for keratitis and conjunctivitis and in topical preparations for skin infections.[1] Gentamicin reaches high concentrations in the kidney and inner ear, so it is essential to monitor for nephrotoxicity and ototoxicity, especially in patients with renal impairment, older adults, those receiving prolonged therapy, and those taking other nephrotoxic drugs or potent diuretics.[1]

"Fortified" means the eye drop is compounded at a much higher concentration than the standard commercial gentamicin drop, which is 0.3% (3 mg/mL). Bayview compounds it at 15 mg/mL, about five times the commercial strength, because a serious eye infection can need more antibiotic delivered to the surface of the eye.
It is used to treat serious bacterial eye infections, most often bacterial keratitis (a corneal infection or ulcer), especially when gram-negative bacteria such as Pseudomonas are involved or suspected. An eye specialist prescribes and monitors it, often based on cultures of the infection.
