Heparin Sodium is an anticoagulant that Bayview compounds to a prescriber's order.
Heparin Sodium is an anticoagulant used to prevent and treat blood clots. It is a glycosaminoglycan that works by binding to antithrombin III, enhancing its ability to inactivate thrombin and factor Xa and thereby preventing clot formation.[1] Heparin is used in managing conditions such as deep vein thrombosis and pulmonary embolism, and for anticoagulation during cardiac surgery, extracorporeal circulation, and dialysis.[1]
Intravenous heparin has an immediate anticoagulant effect, and its terminal half-life ranges from 0.5 to 2 hours depending on dose.[1] Systemic heparin dosing must be monitored with blood tests, such as the activated partial thromboplastin time (aPTT) or anti-factor Xa activity, to reach therapeutic levels while limiting the risk of bleeding.[1]
Heparin cannot be absorbed orally and is given parenterally, by intravenous or subcutaneous routes.[1] Healthcare providers should be aware of potential side effects, including bleeding and heparin-induced thrombocytopenia (HIT).[1] HIT is an immune, antibody-mediated reaction characterized by a fall in platelet count and a hypercoagulable state, and patients may develop thromboembolic complications.[2]
