Nitroglycerin is a nitrate vasodilator that Bayview compounds to a prescriber's order.
Nitroglycerin is a vasodilator that is FDA approved for the acute relief of an attack or acute prophylaxis of angina pectoris secondary to coronary artery disease.[1] It is converted to nitric oxide, which activates guanylyl cyclase and relaxes vascular smooth muscle. Its antianginal effect comes primarily from venodilation, which reduces preload, cardiac workload, and myocardial oxygen demand.[1]
Off-label uses include hypertensive urgency and emergency, coronary artery spasm, congestive heart failure, and chronic anal fissures.[1] After sublingual administration, onset is within 1 to 3 minutes.[1] Nitroglycerin is available in several dosage forms, including sublingual tablets, spray, transdermal patches, ointment, and intravenous infusion.[1][2]
When prescribing nitroglycerin, healthcare providers should be aware of dose-related side effects, including headache, dizziness, and hypotension.[1] Tolerance may develop within 12 to 24 hours of continuous exposure, which is why a nitrate-free or low-nitrate period of 10 to 12 hours a day is used.[2] Use with phosphodiesterase-5 inhibitors such as sildenafil, tadalafil, or vardenafil is contraindicated because of the risk of severe hypotension.[1][2]
