Formula Information

A small arrow pointing down

This formulation combines Amitriptyline HCl 5%, Ketamine HCl 5.75%, and Ketoprofen 10% in a topical gel. Ketamine is an NMDA receptor antagonist that can provide analgesia by reducing central sensitization and pain perception. Ketoprofen is a nonsteroidal anti-inflammatory drug (NSAID) that can reduce inflammation and pain by inhibiting prostaglandin synthesis.

The gel is designed to be applied topically, directly to the area of pain. This allows for targeted treatment and can reduce the systemic side effects often associated with oral medications. The gel is dispensed through a pump mechanism, which allows for easy and precise application. The semisolid formulation ensures that the active ingredients are absorbed through the skin and into the affected area.

This formulation can be used to treat a variety of disease states. The combination of Amitriptyline, Ketamine, and Ketoprofen can provide both immediate and long-term relief from pain and inflammation.

As with any medication, it is important to use this formulation as directed by your healthcare provider. Side effects can include skin irritation at the application site, and systemic side effects can occur if the medication is absorbed into the bloodstream. If you experience any adverse effects, contact your healthcare provider immediately.

It is also important to note that this formulation should not be used in patients with a known allergy to Amitriptyline, Ketamine, or Ketoprofen. Additionally, it should be used with caution in patients with a history of cardiac disease, due to the potential for Amitriptyline to cause arrhythmias.

If you have any questions about this formulation, please do not hesitate to reach out to us at Bayview Pharmacy. Our team of knowledgeable compounding pharmacists is here to assist you.

Drug Interactions

A small arrow pointing down

This compounded formulation contains Amitriptyline HCl, Ketamine HCl, and Ketoprofen, which are all active ingredients that may interact with other medications. It's crucial to inform your healthcare provider about all the medications you're currently taking, including prescription drugs, over-the-counter medications, vitamins, and herbal supplements.

Amitriptyline HCl is a tricyclic antidepressant that can interact with MAO inhibitors, SSRIs, anticholinergic drugs, and certain antihypertensive medications. Combining these medications can lead to serious side effects such as serotonin syndrome, increased heart rate, and elevated blood pressure. It's also important to avoid alcohol while using this medication as it can increase drowsiness and dizziness.

Ketamine HCl is an NMDA receptor antagonist used for its analgesic properties. It can interact with CNS depressants, opioids, and certain antihypertensive medications. These interactions can lead to increased sedation, respiratory depression, and changes in blood pressure. It's also important to avoid alcohol while using this medication as it can increase these side effects.

Ketoprofen is a nonsteroidal anti-inflammatory drug (NSAID) that can interact with anticoagulants, antiplatelet drugs, corticosteroids, and certain antihypertensive medications. These interactions can increase the risk of gastrointestinal bleeding and ulcers, and can also affect blood pressure control. It's also important to avoid alcohol while using this medication as it can increase these risks.

This formulation is used topically, which can reduce the risk of systemic drug interactions. However, systemic absorption can still occur, especially if the medication is applied over large areas of the body, on broken or inflamed skin, or under occlusive dressings. Therefore, it's still important to consider potential drug interactions.

This formulation is typically used to manage chronic pain conditions, such as neuropathic pain, fibromyalgia, and arthritis. Amitriptyline HCl can help to reduce pain signals in the nervous system, Ketamine HCl can block pain signals, and Ketoprofen can reduce inflammation and pain in the tissues. The combined effect of these medications can provide significant pain relief.

If you have any questions about this formulation or its potential interactions, please don't hesitate to reach out to us.

How Does it Work

A small arrow pointing down

This topical gel formulation combines three active ingredients: Amitriptyline HCl, Ketamine HCl, and Ketoprofen. Each of these ingredients works in a unique way to provide relief from various disease states.

Amitriptyline HCl is a tricyclic antidepressant that is used in this formulation for its analgesic properties. It works by increasing the levels of certain chemicals in the brain that help elevate mood and reduce the sensation of pain. When applied topically, Amitriptyline HCl can help to numb the skin and underlying tissues, providing relief from localized pain.

Ketamine HCl is a powerful anesthetic that works by blocking the NMDA receptors in the brain and spinal cord. This action helps to interrupt the transmission of pain signals from the peripheral nerves to the brain, providing a potent analgesic effect. In a topical formulation, Ketamine HCl can provide targeted pain relief without the systemic side effects associated with oral or injectable administration.

Ketoprofen is a nonsteroidal anti-inflammatory drug (NSAID) that works by reducing the production of prostaglandins, chemicals that cause inflammation, pain, and fever in the body. When applied topically, Ketoprofen can provide localized relief from pain and inflammation without the gastrointestinal side effects often associated with oral NSAIDs.

This combination of Amitriptyline HCl, Ketamine HCl, and Ketoprofen in a topical gel formulation allows for targeted treatment of pain and inflammation. The gel is easily applied to the skin and absorbed, providing direct relief to the affected area. This can be particularly beneficial for conditions such as neuropathic pain, arthritis, and other inflammatory conditions.

As compounding pharmacists, we carefully formulate this product to ensure that each ingredient is present in the correct concentration and that the gel is of the right consistency for easy application and optimal absorption. We also take care to ensure that the product is stable and has an appropriate shelf life.

If you have any questions about this formulation, how it works, or how it can be used to treat specific conditions, please don't hesitate to reach out to us. We are here to provide the information you need to make informed decisions about your health and treatment options.

Other Uses

A small arrow pointing down

Apart from its primary function of treating various disease states, the topical gel comprising of Amitriptyline HCl 5%, Ketamine HCl 5.75%, and Ketoprofen 10% demonstrates a broad spectrum of therapeutic properties which makes it a viable option for several other uses. This formulation could be proposed for patients with chronic neuropathic pain syndromes such as postherpetic neuralgia, diabetic peripheral neuropathy, or fibromyalgia. These conditions often require an array of potent, pain-relieving active ingredients that can be efficiently absorbed through the skin and enter the peripheral sites of pain, by means of the gel.

Additionally, this topical gel displays generous anti-inflammatory action, courtesy of the ketoprofen content. This makes it well suited for localized musculoskeletal conditions such as arthritis, strains, or sprains where inflammation is a key component of the disease provoking pain and discomfort. By applying the gel directly on the affected area, the medication provides a localized, targeted treatment which could help reduce systemic side effects.

Furthermore, the amitriptyline in the gel may help improve sleep and mood in patients with chronic pain conditions, given its effects as tricyclic antidepressant. The ketamine inclusion is found to induce rapid-acting antidepressant effects as well, which may be beneficial for patients with major depressive disorder and/or anxiety disorders. The combination of these three active ingredients in one formulation may facilitate a multi-faceted approach for managing these intertwined symptoms.

It's to be noted that every patient is unique and response to therapy may vary. Therefore, it's always recommended to have an open discussion with your healthcare provider to weigh the potential risks and benefits before starting a new medication. Please feel free to reach out to us if you have any questions regarding this formulation or its potential alternate uses.

Compounded preparations have no NDC and will not appear in your drug search. For EPCS to validate, select a drug entry your EHR recognizes as controlled, usually the bulk powder entry for the active ingredient (for example Ketamine (bulk) powder, not a branded product), then put the full formula from step 3 in the prescription body.
