Submitting your first hospice medication order with Bayview Pharmacy is a simple, streamlined process.

You’ll choose the correct CADD order form, enter the required patient and prescriber information, and submit the completed form so our team can prepare the medication and schedule delivery according to your hospice’s rotation.

This guide explains how to pick the right form and complete it accurately to ensure fast, error-free processing.

Choosing the right order form

Bayview provides three standardized medication-specific order forms for hospice CADD pump therapy. Each form gathers the dosing, titration, and clinical details needed for sterile preparation.

Use the appropriate form based on the medication prescribed:

  • Hydromorphone CADD Order Form
    For continuous hydromorphone infusions used in pain and symptom management. Includes standard dosing options and both limited and full titration pathways.
  • Morphine CADD Order Form
    For morphine infusions. Mirrors the hydromorphone layout with stable-dose and titratable-dose sections.
  • Ketamine CADD Order Form
    For ketamine infusions used in complex or opioid-refractory pain. Features simplified dosing fields with provider-directed titration instructions.

Filling out the order form

Each order form follows the same structure. Completing it thoroughly helps prevent delays and ensures uninterrupted therapy for the patient.

Begin by entering the patient information, including name, date of birth, allergies, diagnosis, and the delivery location (home or facility).

Next, add the prescriber details, including printed name, signature, DEA number, and office address. A valid signature is required for all controlled substances.

Select the concentration and bag volume ordered by the prescriber. Available bag sizes vary depending on the medication. Choose the quantity needed—most patients receive two infusion bags at a time, providing a 96-hour supply, unless the care plan requires otherwise.

Enter the infusion instructions, including:

  • Continuous rate (mg/hr)
  • PRN bolus dose and interval, if applicable
  • Titration rules (hydromorphone and morphine only)
  • Any special clinical considerations

Finally, select the necessary supplies such as tubing, infusion sets, or additional accessories. This ensures the nurse has everything required during the visit.

Once all fields are filled out, send the order to Bayview using the method that best fits your workflow.

Once received, our team immediately begins the processing workflow.

What happens after you submit

After the order reaches Bayview Pharmacy, several steps take place to ensure accuracy, sterility, and timely delivery.

A pharmacist first conducts a clinical review, verifying dosing accuracy, titration parameters, bag volume selection, and supply choices. Once approved, your infusion is prepared in our USP <797> sterile cleanroom following all compounding and safety standards.

After preparation, the medication is assigned to your hospice’s delivery cycle so the patient receives their infusion bags on schedule. If clarification is needed at any point, our team will contact you promptly. A confirmation is provided once the order is officially in progress.
