Gabapentin is a versatile active pharmaceutical ingredient commonly utilized in compounded formulations for its efficacy in managing neuropathic pain and certain types of seizures.
Gabapentin is an active pharmaceutical ingredient FDA-approved for postherpetic neuralgia and as adjunctive therapy for partial seizures in adults and children aged 3 and older.[1] It is also used off-label for other conditions, including painful diabetic neuropathy and fibromyalgia.[1] Gabapentin was originally used as a muscle relaxant and antispasmodic, and its anticonvulsant potential was discovered later.[1]
Its mechanism of action involves binding to the alpha-2-delta subunit of voltage-gated calcium channels, which inhibits the release of excitatory neurotransmitters.[1] Although it is structurally similar to GABA, it does not bind directly to GABA receptors.[1]
Common side effects include dizziness, fatigue, somnolence, and peripheral edema.[1] More serious concerns include suicidality, withdrawal seizures if stopped abruptly, and respiratory depression and sedation when combined with opioids.[1] Gabapentin has less than 3% protein binding, undergoes minimal metabolism, and is eliminated primarily by the kidneys, so doses are reduced in renal impairment.[1]
