Tretinoin, a derivative of vitamin A, is a potent active pharmaceutical ingredient widely used in dermatology for its efficacy in treating acne and photoaging.
Tretinoin, also known as all-trans retinoic acid, is a derivative of vitamin A used in dermatology.[1] In acne vulgaris, topical tretinoin normalizes follicular epithelial differentiation, reduces the formation of comedones, decreases inflammation, and promotes turnover of skin cells.[1] Topical tretinoin is also approved as an adjunctive treatment for signs of photoaging, where it has been shown to improve fine facial wrinkles, facial skin roughness, and facial mottled hyperpigmentation.[1]
Tretinoin works by binding retinoic acid receptors (RARs) and retinoid X receptors (RXRs), which act on genes involved in cell differentiation.[1] It also increases procollagen production, supporting the formation of type I and type III collagen.[1]
Topical tretinoin is available in several strengths, with 0.1%, 0.08%, and 0.04% among those commonly prescribed.[1] The most common topical adverse effects include itching, skin pain, irritation, and redness, and patients may be told to use a moisturizer to help with dryness.[1][2] Because therapy increases photosensitivity, patients should use sunscreen, wear protective clothing, and avoid tanning beds and sunlamps.[1][2]
Topical tretinoin is typically applied as a thin layer once daily, preferably in the evening before bedtime.[1][2]
